Accidents and serious incidents which affect the safety of aircraft must, in the first instance, be reported to ATSB by telephone toll-free call, on 1800 011 034 or fax your notification form to: (02) 6274 6434. For further information, follow this link.

A voluntary and confidential aviation notification system was introduced, entitled the Aviation Self Reporting Scheme (ASRS). For information on the scheme and how to make a report, click here.

ATSB receives, assess and acts on confidential reports to prevent or reduce the risks of marine accidents. Reports may be made by telephone, email or fax, or by downloading, completing and posting the reporting form.

Incidents should be notified to AMSA using forms AMSA 18 and AMSA 19. These can be downloaded from the Shipping Industry Forms page accessed by following this link.

REPCON is a voluntary confidential reporting scheme for aviation. REPCON allows any person who has an aviation safety concern to report it to the ATSB confidentially. Protection of the reporters identity is a primary element of the scheme.
