All accidents and incidents involving Australian registered aircraft, or foreign aircraft in Australian airspace, must be reported to the Australian Transport Safety Bureau (ATSB). The ATSB maintains its own database (Occurrence Analysis and Safety Information System) in which all reports assessed by the ATSB as an accident, incident or serious incident are recorded. The Weekly Summary lists those notifications entered into the Bureau's database over the previous week. The data provided is not a reflection of those occurrences reported to the ATSB for the same reporting period.

Note: The Weekly Summary may be read in conjunction with the related Abbreviations, Investigation Categories and Occurrence Log Descriptors. From 8 April 2005 the list of occurrences reported to the ATSB included in the weekly summary contains a brief discription of the reported occurrence.
